Inflammation of the skin (not caused by microorganisms) responding to treatment with corticosteroids when the local treatment with less-acting corticosteroids does not bring the expected results. Follow-up treatment or maintenance treatment for inflammatory skin diseases when prior use of topical potent corticosteroids has produced good results.
Composition:
1 g of cream, lipoxer, emulsion or 1 ml of solution contains 1 mg of Hydrocortisone 17-butyrate.
Action:
Synthetic corticosteroid with moderately strong local effect. It has anti-inflammatory properties and constricts blood vessels. It inhibits inflammatory reactions and alleviates the symptoms of inflammation of the skin with accompanying pruritus, without treatment of the underlying disease. Hydrocortisone butyrate topically applied is absorbed through the skin. The use of an occlusive dressing increases its absorption. It binds to plasma proteins and is hydrolysed to hydrocortisone in the blood and liver. Small amounts are excreted in urine and faeces.
Contraindications:
Hypersensitivity to any of the ingredients. Skin infections: bacterial (eg syphilis or tuberculous lesions), viral (eg chickenpox, herpes, shingles), fungal and parasitic. Skin ulcers, wounds. Inflammation of the skin around the mouth. Acne and rosacea.
Precautions:
Particularly caution should be used in patients with psoriasis due to a relapse due to the development of tolerance, the risk of generalized pustular psoriasis and local and general toxic effects caused by disturbance of skin continuity. Carefully use and avoid long-term treatment in children (risk of suppression of the adrenal cortex and secretion of growth hormone). Do not use on eyelids (when preparations get on the conjunctiva, there is a risk of simple glaucoma or subcapsular cataract), on the face skin, hairy skin (in the case of a solution other than the scalp) and in the genital area. Avoid applying to large areas of the skin, especially under the occlusive dressing, in the folds and folds of the skin - the absorption of corticosteroids may increase, which leads to suppression of the adrenal cortex. Protect eyes and mucous membranes against contact with preparations. The cream and lipokrem contain cetostearyl alcohol (risk of local skin reaction, e.g. contact dermatitis) and propyl parahydroxybenzoate (risk of late-type allergic reaction); the cream also contains butyl parahydroxybenzoate, which can also cause allergic reactions. The emulsion contains cetostearyl alcohol, butylated hydroxytoluene and propylene glycol (risk of local skin reaction, irritation of the eyes, mucous membranes and skin) and propyl and butyl parahydroxybenzoate (risk of late-type allergic reactions).
Pregnancy and lactation:
Corticosteroids penetrate the placental barrier and can act on the fetus. Caution should be exercised when using the preparation during pregnancy - use only if there are clear indications. During breastfeeding, the preparations can be applied to a small area of the skin for a short time. In the case of long-term treatment or application of preparations on a large area of the skin, breastfeeding should be discontinued.
Side effects:
Rare: skin atrophy (often irreversible) with thinning of the epidermis, dilation of tiny blood vessels, purpura, skin striae, pustular acne, dermatitis around the mouth, skin discoloration, dermatitis and eczema (including contact dermatitis), "rebound effect "that can lead to dependence on the topical use of steroids. Very rare: suppressed adrenocortical function. In addition, hypersensitivity was observed. Long-term use of the preparation may result in systemic side effects resulting from adrenal suppression.
Dosage:
A thin layer of cream, lipokrem, emulsion or a small amount of solution 1-3 times daily is applied to the affected skin. After improvement, the product is used once a day or 2-3 times a week.Do not use a dose greater than 30-60 g per week.
