Treatment of increased intraocular pressure (ocular hypertension), chronic form of open-angle glaucoma, glaucoma caused by congenital lack of the lens of the eye or surgical removal of the lens (glaucoma in aphakia). Treatment of other forms of glaucoma, such as: capsule membrane glaucoma, pigmented glaucoma, juvenile glaucoma, hemorrhagic glaucoma.
Composition:
1 ml of solution contains 1.118 mg or 3.354 mg of metipranolol hydrochloride (equivalent to 1 mg or 3 mg of metypranolol).
Action:
A non-selective β-adrenergic receptor antagonist for topical use. The drug reduces intraocular pressure by reducing the production of aqueous humor. Up to 5% of the dose is absorbed by the cornea, the drug has affinity for the structures of the cornea and the inner eye. Methypranolol is a pro-drug, it is rapidly and completely metabolised to biologically active deacetylmetypranolol. T0,5 is 3 hours.
Contraindications:
Hypersensitivity to the components of the preparation. Bronchial hyperreactivity, existing or history of bronchial asthma, chronic obstructive airways disease. Sinus bradycardia, atrioventricular block II and III, symptomatic cardiac failure, cardiogenic shock, decrease in blood pressure, last stages of heart failure. Metabolic acidosis. Degenerative corneal disorders.
Precautions:
In children, the preparation can be used after careful analysis of benefits and risks, and the patient should be under constant observation. Carefully use in patients prone to bronchospasm (eg bronchial asthma).
Pregnancy and lactation:
The preparation can be used during pregnancy (especially during the first trimester), only if absolutely necessary, and the benefits for the mother outweigh the risk to the fetus. Do not use during breast-feeding.
Side effects:
Rare: transient burning after administration, decreased level of tear secretion, short-term reduction of corneal sensitivity, allergic reactions. Very rare: uveitis of the anterior segment of the eye. At the beginning of the therapy, a headache may appear which will disappear after a few days. Patients with a tendency to bronchial spasms (eg bronchial asthma) may develop respiratory failure (bronchospasm). Additional side effects that may occur after the use of the preparation, and were observed after the use of metipranolol in oral form or in higher doses: uncommon - fatigue, pain and dizziness, dizziness, paresthesia, feeling cold in the upper and lower limbs, sleep disorders , muscle spasms and / or their failure; rarely - gastrointestinal disorder, increased myocardial insufficiency, bradycardia, conduction disorders in the heart, decreased blood pressure, impaired delivery of oxygen to the lungs, conjunctivitis, skin reactions, peripheral circulatory disorders, hypoglycemia, changes in blood counts, impaired potency dry mouth; very rarely - depression, hallucinations; isolated cases - a strong attack of angina.
Dosage:
Locally. 1 drop into the conjunctival sac 2 times a day. Treatment should be started with the 1 mg / ml preparation. If the overpressure does not decrease, a higher concentration of the preparation should be used. Depending on the effects of treatment, one dose per day may be used.
