In order to induce immediate and complete narrowing of the pupil during surgical procedures inside the eyeball. Maximum miosis is achieved after a few minutes after administration.
Composition:
1 ml of the solution contains 0.1 mg carbachol.
Action:
A parasympathomimetic drug that causes pupil constriction through a cholinergic effect on the end of motor neurons in the iris sphincter muscle. Strong cholinergic agents cause iris and ciliary shrinkage, which reduces intraocular pressure.
Contraindications:
Hypersensitivity to carbachol or to any of the excipients.
Precautions:
Caution should be exercised in patients with acute heart failure, bronchial asthma, peptic ulcer, hyperthyroidism, disorders characterized by increased gastrointestinal contractility, urinary tract obstruction and Parkinson's disease, and in patients who have had an iritis / uveitis in the past. The use of the preparation may exacerbate postoperative intraocular inflammation (iris congestion). In the event of hypotension, additional reduction of intraocular pressure should be avoided. The stopper of the vial contains natural rubber (latex), which may cause severe allergic reactions. The safety and efficacy of the preparation in children has not been established.
Pregnancy and lactation:
There were no controlled clinical trials in pregnant women. It is estimated that the potential risk is low, taking into account a single administration of a small dose and the site of administration. The preparation should only be used in pregnant women when the expected benefits outweigh the potential risk to the fetus. It is not known whether carbachol passes into breast milk. It should not be used during breastfeeding unless the potential benefit outweighs the potential risk.
Side effects:
Uncommon: headache, increased intraocular pressure, excessive sweating, urgency. Rare: retinal detachment. Not known: visual impairment, corneal retinopathy, corneal opacity, anterior chamber inflammation, corneal edema, inflammation of the eye, prolonged effect of the drug (miosis), blurred vision, eye pain, ocular congestion, vomiting.
Dosage:
Adults. The preparation is for intraocular use only. Aseptically remove the vial from the blister by peeling back the paper that covers it and place it on a sterile tray. Take the contents of the vial into a dry sterile syringe, replace the needle with an atraumatic cannula before administering the drug to the inside of the eyeball. Gently inject not more than 0.5 ml of the preparation into the anterior chamber. The effect of the preparation persists up to 24 h after the procedure. Effective pupil stenosis was observed in some patients even after administering only 5 μg of carbachol. There is no need to adjust the dose in elderly patients or in patients with impaired renal function and (or) liver function.
