Treatment and prevention of recurrences in inflammatory bowel diseases: ulcerative colitis with mild to moderate course; Leśniowski-Crohn's disease, especially when dealing with the diseased process of the large intestine. It can be used in combination with the rectal form of the drug.
Composition:
1 tabl enteral contains 250 mg of mesalazine.
Action:
Salicylic acid derivative (5-ASA) with local anti-inflammatory activity on the intestinal mucosa. The mechanism of action of the drug is not finally understood. It probably involves inhibiting prostaglandin biosynthesis in the mucous membrane of the large intestine. It is believed that mesalazine can act as a so-called scavenger of free oxygen radicals. The drug is slightly absorbed from the gastrointestinal tract. Biotransformation of 5-ASA occurs primarily in the intestinal wall and partly in the liver, where the coupling of mesalazine with acetic acid occurs. This metabolite is excreted in the urine; part of the drug is excreted in the faeces. T0,5 mesalazine is about 1 h, while the acetylated metabolite - 5-10 h.
Contraindications:
Hypersensitivity to mesalazine, salicylates or other components of the preparation. Hemorrhagic sores. Severe renal failure (creatinine clearance less than 20 ml / min) and / or liver. Active peptic ulcer disease. Do not use in children under 12 years.
Precautions:
Use with caution in patients with renal insufficiency, proteinuria and with an increased concentration of non-protein nitrogen (BUN) in the blood. The drug should not be administered simultaneously with agents that reduce the pH in the intestines (eg lactulose). Carefully use with medicines that can adversely affect the hematopoietic system. The preparation contains lactose and should not be used in patients with hereditary galactose intolerance, Lapp lactase deficiency or malabsorption of glucose-galactose.
Pregnancy and lactation:
Use only in case of extreme necessity. Breast-feeding is not recommended during treatment.
Side effects:
Allergic reactions: dermal (urticaria, pruritus, rash, erythema nodosum, acne, skin rash around the anus), acute intolerance syndrome (cramps or abdominal pain, stomach, bloody diarrhea, fever, severe headache, rash), bronchospasm. Pain and dizziness. Nausea, vomiting, diarrhea, abdominal pain. Increase in body temperature, chills, sweating. Very rare: pancreatitis, myocarditis, pericarditis, hepatitis and kidneys, interstitial nephritis, nephrotic syndrome, thrombocytopenia, eosinophilia, leukopenia, agranulocytosis, aplastic anemia, lymphadenopathy, erythema nodosum, reduction in sperm count and motility.
Dosage:
Orally. Adults and children over 12 years of age: in acute relapse - usually 1.5-3 g / day in doses divided by 3-6 weeks; maintenance treatment - 1-1.5 g per day in divided doses (usually 1.5 g per day). The drug should be used about 1 hour before a meal, the tablets should be swallowed whole. In renal dysfunction, the dosage should be modified.
