Action:
Acetylsalicylic acid has analgesic, anti-inflammatory and antipyretic effects. It inhibits the synthesis of prostaglandins by inhibiting arachidonic acid cyclooxygenase. These compounds induce an inflammatory process by leukotactic effects, increased vascular permeability, edema formation, increased sensitivity of the receptors to the action of neurotransmitters (serotonin, bradykinin, acetylcholine). Has analgesic effects in nociceptive pain and neurogenic pain caused by inflammation, in which harmful stimuli stimulate cyclo-oxygenase activity, increasing the production of peroxides and prostaglandins. Acetylsalicylic acid inhibits platelet aggregation. Glycine contained in the preparation reduces the irritating effect of acetylsalicylic acid on the mucous membrane of the stomach which reduces dyspeptic symptoms. Acetylsalicylic acid is absorbed from the digestive tract in 80-100%. Food does not reduce bioavailability, but it may extend the absorption time. Analgesic and antipyretic effects begin after 30 min. from the moment of application, reaching a maximum after 1-3 hours. After a single dose, it is maintained for 3-6 h. Full anti-inflammatory action occurs within 1-4 days of treatment. Acetylsalicylic acid binds to proteins in approximately 33%. It is metabolised in the liver and excreted in the urine in the form of salicylic acid and its metabolites. T0,5 acetylsalicylic acid is 2-3 hours, and salicylic acid is about 6 hours. Acetylsalicylic acid readily crosses the placental barrier.
Contraindications:
Hypersensitivity to acetylsalicylic acid or other ingredients of the preparation and to NSAIDs. Bronchial asthma, chronic respiratory diseases, hay fever or nasal mucosa edema. Active gastric and / or duodenal ulcer, inflammation or gastrointestinal bleeding. Severe hepatic or renal failure. Severe heart failure. Blood coagulation disorders (eg haemophilia, thrombocytopenia) and concomitant use of anticoagulants (eg coumarin derivatives, heparin). G-6-PD deficiency. Co-administration with Methotrexate at doses of 15 mg per week or more, due to myelotoxicity. Children under 16, especially in the course of viral infections, due to the risk of Reye's syndrome. III trimester of pregnancy and breastfeeding.
Precautions:
Do not drink alcohol during treatment with Acetylsalicylic acid, due to the increased risk of damage to the gastrointestinal mucosa. Caution should be exercised in patients with impaired renal function and chronic renal failure, in patients with juvenile rheumatoid arthritis and / or systemic lupus erythematosus and liver failure due to an increase in salicylate toxicity. Monitor liver function in these patients. Caution should be exercised when administering methotrexate at doses lower than 15 mg per week due to increased methotrexate toxicity on the bone marrow, as well as in: uterine haemorrhage, excessive menstrual bleeding, use of the contraceptive intrauterine device, hypertension and heart failure. Elderly patients should be used at lower doses and at longer intervals due to the increased risk of side effects. Acetylsalicylic acid should be discontinued 5-7 days before the planned surgery, due to the risk of prolonged bleeding time during and after the procedure. It may cause fertility problems in women by affecting ovulation, however, this effect is transient and disappears after treatment. In the course of some viral diseases, especially in the case of influenza A or B virus infection, and chickenpox, mainly in children and adolescents, there is a risk of Reyea syndrome - a rare but life-threatening disease.This risk increases if you are given acetylsalicylic acid at the same time. From the above reasons for children under 16 years of age do not use products containing acetylsalicylic acid, and in adolescents over 16 years of age they can only be used if the benefits outweigh the risks. Acetylsalicylic acid, even in small doses, reduces the excretion of uric acid. In predisposed patients, the product may cause gout attacks. The medicine contains 461 mg sodium in 1 effervescent tablet, which should be taken into account in patients with decreased renal function and in patients who control sodium content in the diet.
Pregnancy and lactation:
The preparation in the third trimester of pregnancy is contraindicated, in the first and second trimesters it can be used in cases of extreme necessity. When used in the first and second trimesters, the lowest effective dose should be administered for the shortest possible time. Acetylsalicylic acid can cause damage to the fetal and kidney circulatory system, prolonged bleeding time both in mothers and in newborns and has antiaggregative effects, even when using small doses. It also inhibits the uterine contractility. The preparation is contraindicated during breast-feeding.
Side effects:
Taking the preparation at the lowest effective dose for the shortest time necessary to relieve symptoms reduces the risk of side effects. Dosage:
Orally. Adults and adolescents over 16 years of age: 1-2 tabl. 2-4 times a day. Do not use a dose of more than 3 g of medicine a day. Use the lowest possible effective dose. The product is dissolved in water and taken with food.
