Composition:
1 tabl powl. Gripex contains: 325 mg of Paracetamol, 30 mg of pseudoephedrine hydrochloride and 10 mg dextromethorphan hydrobromide; preparation contains sunset yellow and Allura red AC. 1 tabl powl. Gripex Max contains: 500 mg of paracetamol, 30 mg of pseudoephedrine hydrochloride and 15 mg dextromethorphan hydrobromide; the preparation contains tartrazine lak.
Action:
Combined preparation containing Paracetamol with analgesic and antipyretic effect, pseudoephedrine, which stimulates the α-adrenergic receptors of the smooth muscle blood vessels, resulting in contraction of the end arterioles and reduces nasal congestion and dextromethorphan acting on the center of cough in the medulla prolonged by reducing receptors sensitivity to stimuli from the respiratory tract. Paracetamol is quickly and almost completely absorbed from the gastrointestinal tract. Cmax in the blood reaches on average after 1 hour. It is weakly bound to plasma proteins, in therapeutic doses in about 25%. T0,5 in adults, it is 1.6-8.8 h. The analgesic time is defined as 4-6 h and the antipyretic time is 6-8 h. Approx. 2-4%, excreted unchanged by the kidneys. The main way to eliminate the drug is its biotransformation in the liver. The main metabolite of paracetamol in adults is the combination with glucuronic acid and in children with sulfuric acid. The hepatotoxic indirect metabolite formed N-acetyl-p-benzoquinoline is rapidly coupled to glutathione and excreted in the urine after conjugation with cysteine or mercapturic acid. This mechanism is easily saturated when taking high doses of paracetamol. Hepatic glutathione resources may be depleted causing the accumulation of a toxic metabolite in the liver, which may lead to hepatocyte damage and necrosis and acute hepatic failure. Pseudoephedrine is absorbed quickly and completely from the gastrointestinal tract. Cmax in the serum occurs after 1.5 hours. The decongestant has a decreasing effect after approx. 30 minutes after administration. The maximum effect occurs after 30-60 min. The effect lasts up to 4 hours. It is metabolized with monoamine oxidase. Excreted in 70-90% with urine in unchanged form. Dextromethorphan is well absorbed from the gastrointestinal tract. Cmax in the serum occurs about 2 hours after administration of 30 mg dextromethorphan by the oral route. Antitussive effects occur after approx. 15-30 minutes. and it lasts about 4 hours. Dextromethorphan is metabolized in the liver via N- and O-demethylation followed by binding to sulfate or glucuronide. It is excreted unchanged and metabolites in the urine (up to 56% of the administered dose).
Contraindications:
Hypersensitivity to the active substances or to any of the excipients. Concomitant use of other paracetamol-containing medicines. The use of MAO inhibitors and up to 2 weeks from discontinuation. Congenital G-6-PD deficiency. Severe renal or hepatic failure. Severe hypertension. Ischemic heart disease. Alcoholic disease. Bronchial asthma. Pregnancy and breast-feeding. Gripex should not be used in children under 6 years of age; Do not use Gripex Max for children under 12 years.
Precautions:
The use of the drug by people with liver failure, alcohol abuse and starved poses a risk of liver damage. Caution in patients with renal insufficiency, hypertension, arrhythmia, emphysema, increased intraocular pressure, prostatic hyperplasia, hyperthyroidism, diabetes mellitus and those using anxiolytics, tricyclic antidepressants, other sympathomimetic drugs, i.e. reducing congestion, suppressive Appetite and psychostimulants similar to amphetamines.The drug should be used with caution in people taking anticoagulants. It should not be used for chronic cough with expectoration. The drug should not be used in patients with respiratory failure, bronchial asthma or in patients at risk of respiratory distress. You must not drink alcohol while you are taking the medicine. Gripex tablets contain azo dyes (orange yellow, Allura red AC) that can cause allergic reactions. Gripex Max tablets contain azo dye - tartrazine, which can cause allergic reactions. Patients should be advised that they should consult a doctor if symptoms worsen or do not improve after 3 days.
Pregnancy and lactation:
Do not use this medicine during pregnancy and breastfeeding.
Side effects:
Uncommon: fatigue. Rare: Type I hypersensitivity reactions (allergic edema, anaphylactic reaction), nausea, vomiting, allergic skin reactions, redness of the skin, rash. Very rare: granulocytopenia, agranulocytosis, thrombocytopenia, bronchial asthma, liver damage most commonly due to overdose, renal colic, renal papillary necrosis, acute renal failure, urolithiasis, impaired urination, urinary retention, especially in patients with prostatic hyperplasia . Frequency unknown: hallucinations (especially in children), drowsiness, dizziness, tachycardia, slight increase in blood pressure.
Dosage:
