The drug acts as an analgesic in headaches, throats, migraines, toothaches, bone, joint and muscle pain and menstrual pain. It can be used in colds and flu-like conditions. It works antipyretic.
Composition:
1 tabl powl. contains 500 mg of Paracetamol and 65 mg of caffeine.
Action:
An analgesic and antipyretic medication containing paracetamol in combination with caffeine. Paracetamol has analgesic and antipyretic effects. Inhibits the biosynthesis of prostaglandins by inhibiting arachidonic acid cyclooxygenase at o.u.n. Only slightly inhibits peripheral cyclo-oxygenase enzyme, without causing damage to the gastric mucosa. Paracetamol does not affect platelet aggregation. Caffeine stimulates o.u.n., enhances the analgesic effect of paracetamol and has a weak diuretic effect. Paracetamol is quickly and almost completely absorbed from the gastrointestinal tract and is evenly distributed to body fluids. Plasma proteins bind to a small extent. It is metabolised in the liver and almost completely excreted in the urine in the form of a conjugated - glucuronide and sulfate. A small amount (approximately 5%) of the potentially hepatotoxic intermediate metabolite N-acetyl-p-benzoquinone (NAPQI) is completely conjugated to glutathione and excreted in combination with cysteine or mercapturic acid. If high doses of paracetamol are used, hepatic glutathione stores may be depleted, resulting in the accumulation of a toxic metabolite in the liver. This can lead to hepatocyte damage, necrosis and acute liver failure. Less than 5% of the paracetamol dose is excreted unchanged. Medium T0,5 is 1-4 h. Caffeine is rapidly absorbed after oral administration, maximum concentration reaches within 1 h after administration, T0,5 is about 3.5 h. 65-80% of the administered caffeine is excreted in the urine in the form of 1-methyluric acid and 1-methylxanthine.
Contraindications:
Hypersensitivity to Paracetamol, caffeine or any of the excipients. Alcoholic disease. Severe hepatic or renal failure. Heart arythmia. Insomnia. Children under 12 years.
Precautions:
To be used with caution in patients with: hepatic and renal failure; deficient in G-6-PD dehydrogenase and methemoglobin reductase. While taking the preparation, you should not drink alcohol because of the increased risk of liver damage. The special risk of liver damage exists in patients who are starved and regularly drink alcohol. When using the preparation, excessive consumption of caffeine (eg contained in coffee, tea and some beverages) should be avoided. Do not take other medicines containing paracetamol.
Pregnancy and lactation:
Medicines containing paracetamol and caffeine should not be used during pregnancy due to possible increased risk of spontaneous abortion associated with caffeine intake. Paracetamol and caffeine penetrate into breast milk. Caffeine found in breast milk may potentially have a stimulating effect on a breastfed child, but no significant toxic effects have been observed. The preparation should be used during breastfeeding in case of extreme necessity.
Side effects:
Very rare: thrombocytopenia, anaphylactic reactions, skin hypersensitivity reactions including skin rash, angioneurotic edema and Stevens-Johnson syndrome (bladder erythema multiforme), bronchospasm in patients with hypersensitivity to Acetylsalicylic acid and other NSAIDs, liver dysfunction. Frequency unknown: nervousness, dizziness. If the preparation is administered at the recommended dosage in combination with caffeine contained in food, it is possible to increase the side effects associated with an increase in the total dose of Caffeine, such as: insomnia, restlessness, anxiety, irritability, headaches, gastrointestinal disorders, palpitations.
Dosage:
Orally. Adults and children over 12 years: depending on the severity of the symptoms, 1-2 tabl. up to 4 times a day, not more often than every 4 hours. The maximum daily dose: 4000 mg / 520 mg (paracetamol / caffeine).You should not use it regularly for more than 3 days without consulting your doctor.
