Liquid for use on the skin: topical treatment of mycoses of the skin with or without bacterial superinfection, such as dermatophyte infections (fungi of the genusTrichophyton, Epidermophyton, Microsporum) except for dermatophytosis of the scalp, skin candidiasis, dander versicolor. Healing powder: treatment of onychomycosis caused by dermatophytes (only).
Composition:
1 g of liquid for application to the skin or 1 g of therapeutic powder contains 10 mg of ciclopirox with olamine.
Action:
Antifungal medicine for topical use of the pyridone derivative group. It inhibits the transport of leucine to the fungal cell and damages the cell membrane. By chelating cations of multivalent metals (eg aluminum and iron), it inhibits the action of intracellular enzymes. This leads to the modification of cell metabolism, changes in the structure of the wall and internal structures, and, as a result, to its destruction. Cyclopropox with olamine also inhibits the arachidonic acid cascade, which causes some anti-allergic and anti-inflammatory effects. It works on fungi of the genusCandida (MIC 4 μg / ml) and yeasts, dermatophytes of generaEpidermophyton, Microsporum, Trichophyton (MIC 1-4 μg / ml), and various other fungi (MIC 0.5-8 μg / ml). It also has antibacterial effects on Gram-positive bacteria (staphylococci and streptococcus) and Gram-negative bacteria (Escherichia spp., Proteus spp., Pseudomonas spp., Klebsiella spp., Salmonella spp., Shigella spp., Bacillus spp.). When applied to the skin, it penetrates the epidermis and hair follicles, reaching particularly high concentrations in the stratum corneum. In the case of a fluid for use on the skin, the degree of absorption through the skin is variable; 1.1-1.6% of the dose is present in the urine. The risk of systemic absorption after applying the powder is negligible.
Contraindications:
Hypersensitivity to the components of the preparation.
Precautions:
There are no data on use in children. Do not use acidic soaps during treatment. Avoid contact of the preparation with eyes and mucous membranes.
Pregnancy and lactation:
Do not use during pregnancy. Due to the lack of data on milk penetration, it should not be used during breastfeeding.
Side effects:
Liquid for use on the skin: transient exacerbation of symptoms (burning, erythema, pruritus) in the initial period of treatment, not requiring discontinuation of use; if local hypersensitivity reactions such as blisters on the skin occur, treatment should be discontinued; during long-term use, especially on large surfaces of damaged skin, mucous membranes or under occlusive dressings, systemic side effects can not be ruled out. Healing powder: local reactions (tingling, irritation, erythema, pruritus).
Dosage:
Liquid for use on the skin: fungal skin infection - the preparation is used twice a day, on average for 21 days (from 2 to 4 weeks). Healing powder: onychomycosis - once a day powder is applied to the diseased skin of the toes (an average of 4 weeks). If the lesions do not improve after 4 weeks of treatment, the diagnosis should be verified.
