Treatment of mycoses of smooth, hairy skin, fungal infections of the feet, nails and shins caused by:Trichophyton rubrum, T. mentagrophytes, Epidermophyton floccosum, Microsporum canis, M. gypseum, Candida albicans, C. tropicalis, C. krusei, C. parapsilosis, Cryptococcus neoformans, Aspergillus fumigatus, Malessezia furfur(formerly Pityrosporum ovale). Infections with some Gram-positive and Gram-negative bacteria and mycoplasmas.
Composition:
1 ml of liquid, 1 g of gel or 1 g of suspension contains 10 mg of ciclopirox with olamine. The gel formulation contains: propylene glycol, methyl parahydroxybenzoate and propyl parahydroxybenzoate. The slurry preparation contains: propylene glycol, cetostearyl alcohol.
Action:
Antifungal medicine, a derivative of pyridinone with a wide range of antifungal activity. It works on dermatophytes, yeast-like fungi, molds and mixed fungi. The fungicidal and antiretroviral action of the drug is related to the inhibition of amino acid uptake (leucine) and the accumulation of substrates by cells of significant importance for metabolic processes and growth of fungal cells. Forming chelates with polyvalent cations (Fe3+, Al3+) inhibits the activity of certain enzymes that are involved in the fungal cellular respiration process. The anti-inflammatory effect of the drug is associated with inhibition of cyclooxygenase and lipoxygenase activity. Cyclopropox with olamine also works on some Gram-positive and Gram-negative bacteria, regardless of the pH of the environment and inhibits growthMycoplasma, Trichomonas andChlamydia. The drug penetrates the hair and hair follicles. It penetrates the skin through the epidermis and sebaceous glands; part of the drug remains in the stratum corneum. 0.8-1.6% of the dose penetrates the stratum corneum after 1.5-6 h after application. The concentration of the drug in the skin was 10-15 times higher than the minimum dose that inhibited fungal growth. Ciclopirox and its metabolites are rapidly excreted by the kidneys.
Contraindications:
Hypersensitivity to ciclopirox with olamine or any of the excipients. Fungal inflammation of the eye and the eye area. Do not use in infants and children under 6 years.
Precautions:
Protect eyes from contact with the preparation. The preparation is not intended for the treatment of vaginal mycosis. The gel and suspension due to the propylene glycol content may cause skin irritation. Due to the cetostearyl alcohol content, the suspension may cause a local skin reaction (eg contact dermatitis). Due to the content of methyl parahydroxybenzoate and propyl gel can cause allergic reactions (possible late-type reactions) and exceptionally bronchospasm.
Pregnancy and lactation:
Do not use the product during pregnancy and breastfeeding if it is not absolutely necessary.
Side effects:
The following were observed: burning, skin irritation, erythema, pruritus, edema (in 1-4% of patients).
Dosage:
The product should be applied twice a day to the affected skin, rubbed in lightly or left to dry. Treatment should be continued until the symptoms disappear. The optimal treatment time is 3 weeks. To prevent the recurrence of the disease, it is recommended to continue treatment for up to 10 days after the lesion has disappeared. If the symptoms do not improve after 4 weeks, the patient should contact their doctor again. Do not use dressings and plasters; the exception is onychomycosis. In the onychomycosis, apply the preparation to affected areas and use a closed dressing.
