Acute gastrointestinal contractions: spasms of the esophagus, cardia and pylorus, spastic constipation, overactive biliary dyskinesia. Inflammation of the stomach and duodenum and gastric and duodenal ulcer. The bile colic. acute urinary tract contractions, especially in the course of kidney stones, renal colic, painful pressure on the bladder. Helpful in radiological diagnostics of the digestive tract and urinary tract as well as after surgical procedures. In obstetrics and gynecology: contractions of the birth canal during delivery, puerperitis, removal of the placenta, dysmenorrhegeal menstruation, supportive during abortion (in the initial stage).
The hyoscine n-butyl bromide is a quaternary derivative of scopolamine. It reduces the tone of smooth muscle, weakens gastrointestinal motility and gastric secretion, eliminates biliary and urinary spasms. It is an antagonist of central and peripheral muscarinic receptors and a nicotine antagonist. It has parasympatholytic activity similar to atropine, however, acting peripherally, it has no central symptoms of its activity. It works faster, but shorter than atropine. Hioscine n-butyl bromide penetrates poorly into body fluids, does not penetrate the blood-brain barrier, therefore it does not affect o.u.n. The drug is mainly metabolised in the liver. The degree of binding to plasma proteins is negligible. Excretion occurs mainly in the urine, slightly in the faeces and in the bile.
Contraindications:
Hypersensitivity to hyoscine butyl bromide, atropine or any component of the drug. Glaucoma. Hyperplasia of the prostate gland with urinary retention. Stenosis in the gastrointestinal tract. Tachycardia. Angina pectoris. Heart failure. Colon extension. It is not recommended to administer the drug for 24 h before testing for gastric acid secretion.
Precautions:
Due to the risk of urinary retention or worsening of the clinical condition, special care should be taken in elderly patients with the following diseases: prostatic hypertrophy without obstruction, uropathy from urinary tract obstruction, such as bladder neck obstruction caused by prostatic hyperplasia. Caution is advised when using the drug in patients with mechanical tract obstruction of various origins, pyloric stenosis, widening of the colon - drug administration may lead to stagnation of gastrointestinal content, flatulence and poisoning and, as a consequence, to exacerbate the course of these diseases. Special care should be taken when using the drug in people with heart disease, especially cardiac arrhythmia (tachycardia, tachyarrhythmia). The drug should not be given to patients with closed-angle glaucoma or patients with high fever, because hyoscine butyl bromide has an inhibitory effect on the sweat glands, its use may cause overheating of the body. In patients with symptoms of dry mouth, drying of rhinitis, hypertension and hyperthyroidism, extreme caution should be exercised when using the drug. In the event of any symptoms of hypersensitivity, it is necessary to discontinue the drug. It is not recommended for use in children under 6 years of age.
Pregnancy and lactation:
The preparation can be used during pregnancy only in cases where the benefit for the mother outweighs the potential risk to the fetus or child. If it is necessary to use during pregnancy, caution should be exercised, especially in the first trimester. You should stop breast-feeding while taking the medicine.
Side effects:
The following may occur: increased blood pressure, increased intraocular pressure, tachycardia, heart palpitations, pain and dizziness, ataxia, mydriasis, accommodation paralysis (cycloplegia), lacrimal fluid secretion disorders, bronchial secretion, dry mouth, nausea, vomiting, constipation, urination disorders, urinary retention, pruritus, rash, increased perspiration, redness, tiredness (fatigue), urticaria,anaphylactic shock; very rare: anxiety, agitation, insomnia, nervousness and hallucinations (especially in children and elderly patients).
Dosage:
Intramuscularly or intravenously. Dosage is determined depending on the severity of clinical symptoms and accompanying diseases. Adults: 10-20 mg 2-4 times a day. The maximum daily dose is 100 mg. The preparation can be used in the form of an intravenous drip infusion. Children over 6 years: 5-10 mg 2-4 times a day.
