5 ml of syrup contains 5 mg dextromethorphan hydrobromide. The product contains maltitol.
Action:
Opioid antitussive. Dextromethorphan (a derivative of morphine) inhibits the cough reflex through central action. At therapeutic doses, dextromethorphan hydrobromide does not inhibit the function of the respiratory center, does not cause habituation or addiction. The preparation is well absorbed from the gastrointestinal tract. It is partially metabolized in the liver and excreted unchanged or in the form of a demethylated derivative.
Contraindications:
Hypersensitivity to any component of the preparation. Respiratory failure and risk of its occurrence. Asthmatic cough (simultaneous use of antitussives with bronchodilators is unjustified). Treatment with MAO inhibitors (risk of: high fever, excitability, circulatory collapse). Children under 6 years old. Breastfeeding period.
Precautions:
Before the recommendation of antitussive treatment, one should state the reason for cough, which requires causal treatment, especially in cases of asthma, bronchiectasis, left ventricular failure (regardless of origin), endobronchial or pulmonary embolism, cancer, pulmonary and pleural effusions. If the cough is resistant to a normal dosage of the antitussive, do not increase the dose, but re-evaluate the clinical situation. It is not rational to associate the use of expectorants or mucolytics with antitussive preparations. In the case of liver failure, the dose should initially be halved; as the patient needs and tolerances, the dose can then be increased. Due to the presence of maltitol, the drug should not be used in patients with rare hereditary fructose intolerance.
Pregnancy and lactation:
Large doses of the preparation given at the end of pregnancy may induce respiratory depression in the newborn. Prolonged use in the third trimester of pregnancy, regardless of the dose, may result in a newborn withdrawal syndrome. As a consequence, the legitimacy of each use of dextromorphan during pregnancy must be analyzed. The preparation is contraindicated during breast-feeding.
Side effects:
The following may occur: drowsiness, dizziness, constipation, nausea, vomiting, allergic reactions (rash, urticaria, angioneurotic edema, bronchospasm).
Dosage:
Orally. Children over 6 years: usually 1 mg / kg daily, divided into 4 doses every 6 hours. For example, for a child of the month. 20 kg 1 teaspoon 4 times a day. Do not use a single dose of more than 0.25 mg / kg. Symptomatic treatment should be short-term (up to 5 days). 1 teaspoon (1 scoop) contains 5 ml of syrup (5 mg dextromethorphan hydrobromide).
