indications:
Treatment of a dry, irritating, non-productive cough.
Composition:
1 pellet contains 7.33 mg dextromethorphan; drug contains Glucose (1.1 g) and sucrose (2.2 g). 15 ml syrup contains 20 mg dextromethorphan; drug contains alcohol, sucrose (5.5 g), sodium (1.3 mmol / 15 ml of syrup).
Action:
Antitussive drug. Dextromethorphan is a levodropanol-3-methoxy derivative with antitussive effect. At therapeutic doses, it has no analgesic, depressive effect on the respiratory or psychomimetic system and is characterized by a weak addictive potential. The fully synthetic D-isomer is free of L-isomer with its opioid-like effect. Ciliary medicine is not inhibited by dextromethorphan in therapeutic doses. It is rapidly absorbed after oral administration. Maximum plasma concentration is achieved within 2 hours. Dextromethorphan is metabolised in the liver (first-pass effect). The essential stages preceding conjugation are oxidative O- and N-demethylation. The main active metabolite is dextrorphan. The fraction excreted by the kidneys within 48 h after oral administration constitutes 20-86% of the applied dose. Less than 1% is detected in faeces. T0,5 in the phase of elimination it is 1.2-2.2 h, but it may extend to 45 h in the case of abnormal metabolism. The onset of action occurs within 15-30 min after oral use, and the effect persists for approximately 3-6 hours.
Contraindications:
Hypersensitivity to dextromethorphan or other components of the drug. Bronchial asthma, chronic obstructive pulmonary disease (chronic bronchitis and emphysema), pneumonia, respiratory failure, respiratory depression. Concomitant use of MAO inhibitors or within 14 days after discontinuation of MAO inhibitors. Tablets do not use in children under 12 years. Do not use syrup in children under the age of 14 years. Severe liver dysfunction.
Precautions:
The medicine can only be used after very careful consideration of the benefits and risks in case of liver dysfunction and in the case of productive cough with significant mucus production. Chronic cough may be an early symptom of bronchial asthma, a drug not indicated in relieving such cough, especially in children. Dextromethorphan has little addictive potential; drug tolerance and psychological and physical dependence may develop during long-term use. In patients with a tendency to abuse drugs or addiction to medicines, the drug should be used for a short period and under close medical supervision. The pellets contain sucrose and Glucose, the syrup contains sucrose, which should be taken into account in patients with diabetes and should not be used in patients with rare, inherited fructose intolerance, glucose-galactose malabsorption or sucrase-isomaltase deficiency. Syrup also contains sodium, which should be taken into account in patients on a controlled sodium diet and 5% alcohol (0.63 g / 15 ml), which should be taken into account in patients with liver disease, alcohol, epilepsy, organic brain disease , pregnant women, breastfeeding women and children.
Pregnancy and lactation:
The use of the drug during pregnancy and breastfeeding should be done after careful consideration of the benefits and risks and only in exceptional cases. Large doses of dextromethorphan may cause respiratory depression in newborns, even if used temporarily.
Side effects:
Uncommon: hypersensitivity reactions, including anaphylactic shock, shortness of breath and swelling of the mouth and throat; moderate fatigue, dizziness; nausea, gastrointestinal problems, vomiting. Rare: skin rash. Very rare: drowsiness, hallucinations, addiction development, in case of abuse.
Dosage:
Orally. Pills, adults and adolescents aged> 12 years: if necessary, 2 lozenges every 4-6 h; the maximum daily dose is 12 pellets. Syrup, adults and adolescents aged> 14 years: 15 ml 3 times a day; the maximum daily dose is 45 ml. Self-treatment should be limited to 3-5 days.
