Some cases of peptic ulcer of the stomach and duodenum and reflux oesophagitis. The Zollinger-Ellison team.
Composition:
1 enteral capsules contain 40 mg of omeprazole.
Action:
Proton pump inhibitor (H+/ K+ATP-ases) in the parietal cells of the stomach. It inhibits the secretion of hydrochloric acid, both basic and stimulated (regardless of the type of stimulus). Increases pH and reduces the volume of secreted gastric juice. At pH <4, Omeprazole is transformed into a more active form - omeprazole sulfonamide. After oral administration, omeprazole is absorbed in the small intestine. The maximum concentration in the blood occurs within 1-3 h after administration. T0,5 is on average 40 minutes. About 95% is bound to plasma proteins. It is completely metabolised, mainly in the liver by the CYP2C19 enzyme. The single-dose bioavailability is about 35% (food does not affect bioavailability). Bioavailability is increased to about 60% after the Next dose and can reach up to 90% in patients with hepatic impairment. Almost 80% of the dose is excreted in the form of metabolites in the urine, the remaining 20% - with faeces. In patients with impaired renal function, elimination of the drug is reduced to a degree consistent with renal function impairment. In patients with hepatic impairment the elimination is reduced, the half-life can be up to about 3 hours.
Contraindications:
Hypersensitivity to omeprazole or other components of the preparation. Patients with liver failure treated with clarithromycin. Do not administer concomitantly with atazanavir. Do not use in children under 2 years.
Precautions:
Caution should be exercised in combination therapy in patients with hepatic and renal impairment. Due to the sucrose content of the preparation should not be used in patients with fructose intolerance, malabsorption of glucose-galactose and sucrase-isomaltase deficiency.
Pregnancy and lactation:
The preparation may be used during pregnancy and breastfeeding only in cases where the expected benefit to the mother outweighs the potential risk to the fetus or breast-fed child.
Side effects:
Common: drowsiness or insomnia, dizziness and headaches, fatigue, diarrhea, constipation, flatulence (also with abdominal pain), nausea, vomiting. Uncommon: visual impairment (blurred vision, decreased visual acuity and visual field), hearing disorders (eg tinnitus), taste disturbances, changes in liver enzymes, pruritus, skin eruptions, hair loss, erythema multiforme, photosensitivity, increased sweating, peripheral edema. Rarely: non-pigmented anemia, microcytic anemia, paresthesia, emptiness in the head, disorientation, hallucinations especially in seriously ill or elderly patients, brown-black language during simultaneous use with Clarithromycin and benign glandular cysts, weakness in muscle strength, muscle pain and joints. Very rare: changed number of blood cells, transient thrombocytopenia, leukopenia, pancytopenia, agranulocytosis, urticaria, elevated body temperature, angioneurotic edema, bronchospasm, anaphylactic shock, allergic vasculitis, fever, agitation and depressive reactions, especially in critically ill or elderly patients , dryness and inflammation of the oral mucosa, candidosis, pancreatitis, hepatitis with or without jaundice, hepatic failure and encephalopathy in patients with pre-existing severe liver disease, Stevens-Johnson syndrome, toxic epidermal necrolysis, interstitial nephritis, hyponatremia, gynecomastia.
Dosage:
Orally. Adults - some cases: duodenal ulcer: 40 mg once daily for 2-4 weeks; gastric ulcer: 40 mg once a day for 4-8 weeks; reflux oesophagitis: 40 mg once daily for 4-8 weeksZollinger-Ellison syndrome: individually, there is no prescribed maximum period of use of the drug, the initial dose of 60 mg once a day, a daily dose of more than 80 mg should be given in two divided doses. Capsules should be taken on an empty stomach before a meal, swallowed whole with a glass of water. The capsules should not be chewed or chewed. In case of swallowing difficulties, the capsule can be opened and its contents swallowed with water; you can also dissolve the contents of the capsule in a small amount of fruit juice or yogurt.
