Short-term symptomatic treatment of constipation, including habitual and chronic constipation in bed-ridden patients and the elderly. Preparation for diagnostic tests, surgical and obstetrical procedures as well as in the perioperative period.
Composition:
1 suppository contains 10 mg of bisacodyl.
Action:
A laxative that stimulates the peristalsis of the large intestine as a result of irritation of the intestinal mucosa or direct stimulation of the nerve endings of the submucosal and muscular plexus. Bisacodyl is poorly absorbed from the intestines and impedes the absorption of electrolytes. Thus, it increases the water content in the lumen of the intestine by osmosis, which results in softening the stool, which facilitates its movement through the large intestine. The volume of fecal masses also increases, which stimulates intestinal peristalsis, facilitates defecation and cleansing of the intestine, eg before diagnostic tests. Laxative action occurs about 20-45 minutes after application. Colorectal bacteria enzymes metabolize bisacodyl to a therapeutically active diphenol, which in the first-pass effect is coupled with glucuronic and sulfuric acid, in this form, with bile, to return to the intestine, thereby undergoing enterohepatic circulation, prolonging the effect of bisacodyl.
Contraindications:
Hypersensitivity to bisacodyl or any of the excipients. Bowel obstruction. acute inflammation within the abdominal cavity, including appendicitis and acute inflammatory bowel disease. Acute abdominal pain that occurs simultaneously with nausea and vomiting. Severe dehydration. The preparation should not be used in patients with an anal fissure or with anal or rectal inflammation accompanied by ulceration or mucosal damage.
Precautions:
The use of bisacodyl should not last longer than 5 days without thoroughly investigating the cause of intestinal obstruction. Prolonged use of the drug may lead to habituation and to electrolyte imbalance and hypokalemia. Intestinal fluid loss can lead to dehydration of the body, which is manifested by increased thirst and oliguria. The use of bisacodyl should be discontinued in patients in whom dehydration may be particularly harmful (in patients with renal insufficiency or in elderly patients). Treatment can be resumed only under close medical supervision. There have been reports of dizziness and / or fainting during defecation, which were caused by increased pressure of the abdominal press, or constituted the vasovagal response of the body to subcutaneous pain caused by constipation.
Pregnancy and lactation:
Bisacodyl should not be given to pregnant women, especially in the first trimester, unless in the doctor's opinion the benefit of the medicine outweighs the potential risk to the fetus. During the long-term use of Bisacodyl, unfavorable or damaging effects on the fetus have not been demonstrated. Do not use the drug during breastfeeding, unless in the doctor's opinion the benefit of the medicine outweighs the potential risk to the child. Both the drug and its glucuronide derivatives do not penetrate into breast milk.
Side effects:
Common: cramps in the abdominal cavity, abdominal pain, diarrhea, nausea. Uncommon: dizziness, blood in the stool, vomiting, abdominal discomfort, irritation of the anal area. Rare: anaphylactic reactions, angioneurotic edema, hypersensitivity reactions, dehydration, fainting, colitis. In individual cases, as a consequence of the use of bisacodyl, blood diarrhea and abdominal pain were observed. In some cases, there was association with colonic mucosal ischaemia.
Dosage:
Rectally. Adults.Short-term treatment of constipation, including habitual and chronic constipation, atonic constipation: 1 suppository once a day, ad hoc. Use no longer than 5 days.Preparation for diagnostic tests and surgical procedures.
