indications:
Auxiliary anesthesia. Blockage of peripheral nerves.
Composition:
1 ml of a 10 mg solution of procaine hydrochloride.
Action:
Locally anesthetic agent with ester structure. It is not absorbed from mucous membranes, therefore it is not suitable for surface anesthesia. The mechanism of action is based on the reversible blocking of sodium channels. Na o.u.n. local anesthetics act initially stimulant, then there may be inhibition of activity o.u.n., sometimes ending with death due to respiratory paralysis. In the circulatory system, local anesthetics reduce the conduction, contraction force and excitability of the myocardium, prolong the refractory period, and expand small arteries at higher concentrations. After the injection, the effect of the drug occurs after 2-5 min and lasts 1-1.5 h. Procaine quickly hydrolyzes.
Contraindications:
Hypersensitivity to procaine hydrochloride, other para-aminobenzoic acid esters or other components of the preparation. Myasthenia gravis. Severe shock or conduction in the heart. Local infection or inflammation at the application site. Allergy (asthma, urticaria).
Precautions:
Do not inject intravascularly. When using procaine, equipment for patient monitoring and resuscitation as well as necessary medication should be provided. Use with caution in patients with weakness, hypotension, hypotension, congestive heart failure, kidney disease, liver dysfunction and reduced blood pseudocholinesterase activity. Caution should be exercised when administering procaine hydrochloride to epinephrine in elderly patients with cardiovascular disease, diabetes mellitus, hyperthyroidism or Graves-Basedov disease. Because of the risk of ischemia, epinephrine should not be used together with procaine during anesthesia of the fingers, nose, eyes and penis.
Pregnancy and lactation:
There are no data on the safety of the medicine in pregnant women. Do not use during breast-feeding.
Side effects:
Hypersensitivity reactions (urticaria, face edema, rash, anaphylactic shock), anxiety, irritability, breathing disorders, paralysis, unconsciousness, muscle tremors up to generalized seizures, headache and dizziness, paresthesia, paraplegia, nausea, vomiting, depression blood pressure, tachycardia, bradycardia, ECG changes (within the T wave, ST segment shortening), chest pain (when concomitant use of a vasoconstrictor), peripheral vasodilation, renal dysfunction, local reactions (burning, pruritus, edema) , redness, superficial or deep necrosis, hematomas, follicular-follicular changes), fever, hypothermia, anemia, impotence, incontinence and / or defecation, methemoglobinemia, numbness of the lips, difficulty in opening the mouth.
Dosage:
The dose depends on the type of anesthesia, the required level of anesthesia and the patient's response to the medicine. The smallest doses and solutions in the smallest concentrations should be used. Auxiliary anesthesia: adults - a single dose is 350-600 mg, use a solution of 0.25% or 0.5% (after diluting 1% solution); procaine may be used concomitantly with epinephrine at a concentration of 1: 200,000 to 1: 100,000; children - the usual dose is 15 mg / kg, use a 0.5% solution. Peripheral nerve blockage: adults - dose is 500 mg, use 0.5% or 1% solution. In adults, do not exceed the initial dose of 1g. In patients with impaired hepatic function, cerebrovascular disease, occlusive disease and elderly patients, the dose should be reduced.
