Source of fats in parenteral nutrition of patients in whom oral or enteral nutrition is impossible, insufficient or contraindicated.
Composition:
1000 ml of emulsion contains 60 g of purified soybean oil, 60 g of medium-chain triglycerides, 50 g of purified olive oil, 30 g of fish oil. Energy value 1000 ml: 8400 kJ (2000 kcal). Osmolarity: about 380 mOsm / l; pH around 8.
Action:
Fatty emulsion for parenteral nutrition. It contains high-energy compounds, provides essential fatty acids.
Contraindications:
Hypersensitivity to egg, fish, soy and peanut protein or other ingredients. Severe hyperlipidaemia, severe hepatic failure, severe blood clotting disorders, severe renal failure without the possibility of hemofiltration or dialysis, acute shock, acute pulmonary edema, conduction, decompensated heart failure, unstable patient condition (severe post-traumatic condition, uncontrolled diabetes, acute myocardial infarction, stroke, embolism, metabolic acidosis, severe sepsis, hypotonic dehydration). It should not be used in children.
Precautions:
If adverse reactions occur or if the triglyceride concentration during infusion exceeds 3 mmol / l, the infusion should be discontinued or, if necessary, continued at lower doses. Cautiously used in renal failure, diabetes, pancreatitis, impaired liver function, hypothyroidism and sepsis.
Pregnancy and lactation:
Due to the lack of appropriate clinical trials, the preparation can be used during pregnancy and breastfeeding only if the anticipated benefits to the mother outweigh the potential risk to the fetus.
Side effects:
Often, a slight increase in body temperature may occur. Uncommon may occur: lack of appetite, nausea, vomiting and chills. Rarely observed: hypertension or hypotension, dyspnoea and hypersensitivity reactions (rash, urticaria, sudden facial redness, headache and anaphylactic or anaphylactoid reactions - indication for immediate interruption of the infusion), feeling cold or hot, paleness, cyanosis, neck, back, bones pain and the lumbar area, and very rarely, priapism. Patients with reduced fat burning capacity, renal impairment or infection may experience "fat overload syndrome" (hyperlipidemia, fever, fat infiltrates, hepatomegaly with or without jaundice, splenomegaly, anemia, leukopenia, thrombocytopenia, coagulation disorders, haemolysis, reticulocytosis) , abnormal results of liver function tests, coma).
Dosage:
Intravenously (into the central vein or peripheral veins) by drip infusion; dosage and infusion rate depending on the ability to eliminate fats. Adults: usually up to 1-2 g fat / kg / day. Infusion rate: 0.125 g fat / kg / h. The maximum dose: 0.15 g fat / kg / h.
