GENERAL TREATMENT
Groups of drugs used in general treatment:
- antibiotics
- retinoids
- sulphones
- hormonal medicines
- other, non-standard methods of acne treatment
antibiotics
Antibiotics owe their effectiveness to destroying the above-mentioned Propionibacterium acnes bacteria and anti-inflammatory effects. For years, antibiotics of choice are tetracyclines of the first and second generation. Second generation tetracyclines (limecycline, doxycycline and minocycline) are distinguished by higher therapeutic efficacy. In addition, other antibiotics may be used - e.g. erythromycin and clindamycin. Antibiotic therapy should avoid solar UV radiation and phototherapy, due to the risk of sensitization and phototoxic reaction.
sulfones
In cases of acne resistant to antibiotic therapy, in which cyst eruptions occur, if treatment with isotretinoin is impossible, the so-called sulfones that have bacteriostatic and anti-inflammatory effects.
COMBATING STRONG ŁOJOTOKU
The strong anti-seborrhoeic effect is primarily hormonal drugs and retinoids (the action of the latter discussed earlier)
Hormonal control of severe seborrhea
Hormonal therapy is used in people who are ineffective treatment with antibiotics and in those who have symptoms of hyperandrogenism, and skin changes clearly exacerbate during menstruation.
Cyproterone acetate and ethinyl estradiol
Hormones are the basic drugs that suppress the synthesis of androgens and reduce the production of sebum.
spironolactone
Hormone also inhibits androgen synthesis - but it is given less often, usually to women over 30 years of age. Unfortunately, it can cause symptoms of menstrual disorder and painfulness of the breast glands.
drospirenone
Hormone is a modern progestogen used in combined contraceptives. It has beneficial anti-androgenic activity and effectively inhibits the activity of male hormones. The result is a significant reduction in the severity of acne symptoms.
corticosteroids
Hormones inhibit the production of androgens by the adrenal cortex. They can be used in women with hyperandrogenism (prednisone) in combination with estrogens. In men, corticosteroids are generally administered only in fulminant acne with general symptoms in the form of elevated temperature and seizure of the joints.
Retinoids used generally
- the most effective treatment, reserved for the most severe forms of acne. Isotretinoin (13-cis Wit A acid) is used. The treatment lasts several months. May cause excessive drying of the skin and mucous membranes. The treatment lasts for several months and must be carried out under close medical supervision. Women must use effective contraception throughout the treatment period and 2 months after the end of treatment. Patient's written consent for this treatment is required. Retinoids used in general are the most effective treatments, reserved for the most severe forms of acne.
Optimal results of acne treatment
Optimal results of the treatment of chronic acne vulva are achieved by applying topically and generally a complex of several drugs. In topical treatment - antibiotics, azelaic acid, benzoyl peroxide and retinoids - so as to simultaneously inhibit: 1) overproduction of sebum in sebaceous glands of hair and skin; 2) keratosis and clogging of the mouths of these glands; 3) multiplication of bacteria that break down tallow into irritating fatty acids; 4) reduce inflammation.
